GPA Entry Checklist
IV. Entry ChecklistBefore mailing your submission, review this checklist to ensure your entry is complete:
- An official entry form must be completed for each regular entry and sent with your entry DVD(s).

- A purchase order does not constitute payment. Entrants whose payments are late will not have their entries sent to the judging site and will be disqualified.
- Entry forms must be completed and mailed with your DVD to NATOA Headquarters. An entry form must accompany each entry, and be securely attached to the disk (with a rubber band). If entry forms are completed on-line but payment is made by check, be sure to print a copy of the on-line entry form and include with your payment.
- Each entry must be on a separate DVD and properly labeled (name of jurisdiction, governmental entity or organization, category entered, running time and program title).
- Disks will not be returned (no masters please).
- Follow rules and guidelines that pertain to the section that the entry is being made in. Failure to comply may result in disqualification.
